    Community Health Needs Assessment
    Sutter Solano Medical Center

Sutter Solano Medical Center has a long tradition of working to improve the health of the communities we serve through our community benefit activities and programs. These efforts are based on needs identified through a community health needs assessment, conducted once every three years, in collaboration with other health care providers, public health departments and a variety of community organizations.

    Community Health Needs Assessment

In 2016, Sutter Solano Medical Center conducted a community health needs assessment (CHNA) to identify the significant health needs in the communities we serve.

    Implementation Strategy

The CHNA was used to develop Sutter Solano Medical Center 2016 – 2018 implementation strategy, a plan that demonstrates how our hospital will work to address the significant health needs in our community.
